New Patients at COR Spine

COR Spine and Pain Center provides individualized, non-surgical pain management focused on helping patients improve comfort, function, and quality of life.  We understand navigating your first visit with a new doctor can be intimidating.  Whether you are visiting our Back Express Walk-in Clinic or have an appointment with Dr. Bruce Witmer, our goal is to make your first experience as smooth as possible.

What can I expect during my first visit?

  • Fill out New Patient Forms

    • Forms available below or in our office​

  • Provide reception your valid photo ID & insurance card

  • Initial payment is required at check-in

  • Our Medical Assistant will go over your medical history & medication list

  • You will then meet with Dr. Witmer or our Advanced Nurse Practitioner. They will discuss your history & review any recent images of your injury.

  • If your X-rays are older than 6-months at the time of your visit or none are available, new images may be ordered at the time of your visit.

  • An individualized recommended course of treatment will be discussed with you during your visit.
